Send a Case

1. Verify the impressions for accuracy and quality. Please follow safety guidelines and pack your sanitized impression and bite in a biohazard bag, or a sanitary zip-type baggie.  If you are sending models, PLEASE WRAP IN BUBBLE WRAP before placing them in the bag or they will break in transit! Any small parts put in vial or envelope. 

2. Complete and sign the prescription form. Don’t forget material choice and shade!  Please attach to the OUTSIDE of the biohazard bag.

3. If you have multiple cases in a box, please staple the Rx to the matching impression. Please DO NOT put the prescription inside the bag! You can send as many cases as will fit in one box, and as many boxes as will fit in one bag using just ONE label.

4. Follow our RETURN DATE CALENDAR for the accurate date the case will be back in your office. The Return Date Calendar has two dates in each box, the date you are sending the case (today's calendar date) the second date in bold is the date on or before the case will be back in your office!*

5. Schedule the PICKUP with UPS/FedEx online, if you don’t have an account yet please call and we will set your office up in our system! Place the case boxes in the shipping bag and attach the shipping label to the bag.  

6. If you have photos to send to us, please email them to info@moderndentalusa.com and reference the Doctor and Patient names so we can match them to the case.
